Operators ran a safety test on Reactor 4 with most safety systems manually disabled, pushing the reactor into a state its own designers had warned was unstable — because the test had been ordered, delayed, and had to be completed on this shift. A known design flaw in the control rods (classified secret, hidden even from the operators) turned the emergency shutdown itself into the detonator. The state then spent days denying what dosimeters were screaming.
